Example:   if your account number is 000819 000, enter only the 819 as your account number.

 

DEBIT MY CHECKING ACCOUNT:   To authorize the Authority to automatically deduct your quarterly sewer payment from your checking account, we must have your signature and a voided check per the instructions on the form below.   Please print the form, fill it out as indicated and return to us with a voided check.   The form (and the check) may be mailed, delivered, faxed or scanned to us in return.  The authorization and deduction will occur the next billing cycle.  Please note that as rate increases become effective, the amount of the deduction will be adjusted to match the standard rate.   If you provide your email address, we will confirm each payment that is deducted by email to your computer.   All information such as phone number and email addresses are kept strictly confidential - for internal use only.
